Policies 

Policies:

1. Must sign consent forms.

2. Running Late ? Make sure to give me a call before hand.

3. No kids or a plus one allowed during your appointment.

4. Your appointment will be canceled once you're over 10 minutes late of your scheduled time and will have to reschedule for another time. ($25 cancellation fee will be required in order to be placed back on the schedule)

5. Please use the restroom before your appointment Time.

6. Must be 18 & older to have teeth whitened and tooth gems placed.

7. Non refundable deposit (no exceptions )

8. Your entire appointment will take a hour and a half to two hours due to my prep work and after care. 

9. Please wait at least three weeks after getting your braces removed to book your teeth whitening appointment!

10. you can not receive this service if you are pregnant!

11. Must pay a $25 cancellation fee if you cancel your appointment in less than 24 hours of your scheduled appointment time ( no shows included)

12. you have two opportunities to reschedule your appointment until your deposit is no longer valid. ( after two times rescheduling you will have to pay another deposit )
